Before Your First Session
Please download, complete, and bring the required forms to your first in-water training session. The Medical Statement and Liability Release are required for all in-water programs. Additional forms are needed depending on your specific course — see the sections below.
Required for All In-Water Programs
Every student must complete and bring both of these documents.
📋 Medical Statement
The RSTC Medical Statement (Diver Medical Participant Questionnaire) is required for all in-water training. Review, complete, and sign before your first session.
📄 Liability Release
The Liability Release and Assumption of Risk Agreement is required for all in-water training programs. Read carefully, complete in full, and bring a signed copy to your first session.
Open Water Diver Course Forms
These additional forms are required specifically for the PADI Open Water Diver Course.
Safe Diving Practices Statement
Standard Safe Diving Practices Statement of Understanding — required for Open Water Diver students.
OW Course Record & Referral
PADI Open Water Diver Course Record and Referral Form — required for Open Water Diver students.
Other Forms
Additional forms for specialty programs, boat travel, and special events.
Discover Scuba Registration
Registration form with Medical Statement & Liability Release — must be completed prior to your Discover Scuba Experience.
Enriched Air Dive Planning Worksheet
Required for Nitrox / Enriched Air dives. Complete prior to diving with EANx.
Boat Travel & Trip Waivers
Liability releases for boat travel, special events, certified diver experience programs, travel & excursions, and snorkeling.
